Nursing care of 2 cases of lung transplantation patients with graft-versus-host disease after allogeneic hematopoietic stem cell transplantation

To summarize the nursing experience of 2 cases of lung transplantation in patients with versus host disease after allogeneic hematopoietic stem cell transplantation. Nursing points include observation and prevention of rejection reaction;strictly implementing prevention and control measures to reduce the risk of infection;through various rehabilitation training,promoting the recovery of respiratory function;conducting nutritional screening and developing nutritional support programmes;timely assessment of psychological state,individual psychological counseling. With aggressive treatment and care,2 patients recovered well and were transferred out of the intensive care unit.

To summarize the first aid and nursing experience of an ICU patient with stress cardiomyopathy after kidney transplantation. Nursing points:focus on changes in the condition and implement efficient first aid measures; rapid and accurate assessment,safe and stable transport;strict hemodynamic measurement,goal-oriented fluid management;precise drug administration,observation of drug effects and adverse reactions; emphasis on psychological counseling and rehabilitation training and doing a good job of continuous nursing are keys to the smooth transfer of patients. After careful treatment and care,the patient was transferred to the kidney transplant ward for continuous specialist treatment on the 16th day after the operation. On the 38th day after the operation,he recovered and was discharged from the hospital.

Application of modified citrate infusion method for patients under continuous renal replacement therapy

Objective The aim of this study is to compare the anticoagulant effects of different citrate infusion methods in treatment with continuous blood purification(CBP). Methods From January to December 2018,40 patients with CBP were recruited randomly from Intensive Care Unit of the Affiliated Hospital of Qingdao University. These patients were randomly allocated into 2 groups according to the different infusion methods of citrate. In the control group,patients(n=20) were treated with a simplified infusion method,and all 4% citrate solution was infused into the artery of the single needle double lumen tube. In the observation group,patients(n=20) were treated with the improved citrate infusion method. Briefly,4% sodium citrate solution was infused into the artery and venous chamber,respectively. The sum of 2 pumping speeds was the same as that in the control group. The coagulation levels of filters and venous chambers in 2 groups after 48 hour were compared respectively. The monitoring was terminated when the level Ⅲ coagulation presented in venous chambers. The peripheral calcium,filtered calcium,bicarbonate value and the service life of cardiopulmonary bypass were measured. Results After 48 hours,40% of venous chambers in the control group presented level Ⅱ coagulation,which was significantly higher than 10% in the observation group(P=0.031) with none of tube presenting level Ⅲ coagulation. The average service time of cardiopulmonary bypass in the observation group was(96.78±17.49) h,which was significantly higher than(65.50±13.63) h in the control group,and the differences were statistically significant(t=12.204,P=0.001). There was no statistically significant difference in coagulation status and blood calcium value between 2 groups(P>0.05). Conclusion The modified regional citrate infusion is a safe,feasible,and effective method for the treatment of CBP.

Investigation and analysis of factors related to hyperglycemia in ICU patients undergoing enteral nutrition

Objective To investigate the status of hyperglycemia during enteral nutrition therapy in non-diabetic ICU patients and explore the related risk factors.Methods Relevant information of 176 non-diabetic patients who received enteral nutrition from January 2018 to September 2018 in two ICUs of a tertiary hospital in Qingdao was retrospectively analyzed adopting case control study design.Results Thirty-seven patients developed hyperglycemia,and the incidence was 21.02%. Patients with and without hyperglycemia were statistically different in age(OR= 1.12),APACHE Ⅱ score(OR=1.41),nasal feeding speed(OR=1.15),the type of nutrient solution(OR=0.45),parenteral nutrition(OR=3.01),antibiotics(OR=1.77),mechanical ventilation(OR=7.16),surgery(OR=43.99),and feeding pathway(OR=6.11).Conclusion Age,higher APACHE Ⅱ score,increased nasal feeding speed,short peptide and haplotype enteral nutrient solution,parenteral nutrition,mechanical ventilation,surgery,application of nasointestinal tube are risk factors of hyperglycemia during enteral nutrition therapy in non-diabetic ICU patients.
